We can currently consider referrals for the following services:

Ketogenic therapy in person in Melbourne;
Ketogenic therapy via telehatlh (video or phone call) across Australia;

We cannot currently consider referrals for the following services:

Face to face therapy in rural NSW and VIC;
Functional capacity assessment (FCA) report;
Sensory processing assessment report;
Group therapy sessions;
Supported independent living (SIL) assessment report;
Specialist disability accommodation (SDA) assessment or report;
Home modifications (Home Mods) assessment or report;
Assistive Technology (AT) assessment or report.

PLEASE READ THE POINTS BELOW IN FULL BEFORE COMPLETING THIS REFERRAL FORM:

- We can provide Occupational Therapy to Privately Funded Clients and NDIS Participants whose NDIS Plans are Plan Managed;

- We WILL NOT accept any referrals in relation to Self or Agency managed NDIS participants;

- If you state 'private funding' but are referring to NDIS Self Managed funding, the referral will be rejected;

- We can provide face to face services when in local proximity for regional VIC and regional NSW clients;

- Completion of this referral form DOES NOT guarantee a referral will be accepted.
